fix build with SQLITE_ENABLE_UNLOCK_NOTIFY

Browse Source 
Move extern declarations for _sqlite3_*_blocking functions before
_sqlite3_exec_no_args which references them. Remove unused
_sqlite3_prepare_v2_nolen function.
